A 29-year-old male presents to the emergency department due to chest pain 

  • Pain began after being tackled in a football game. He was struck over the right side of the chest
  • He also complains of shortness of breath, dysphagia, and paresthesia of the right upper extremity 
  • Vital signs are T 99 F, HR 120 bpm, BP 128/70 mm Hg, RR 30/min
  • On examination he appears to be in moderate distress. Swelling and tenderness are noted over the right side of the chest. There is reduced range of motion of the right upper extremity.

What is the most likely diagnosis and its management?

Answer

Diagnosis: Posterior sternoclavicular dislocation
Management: Urgent closed reduction

Explanation

This patient is presenting with dyspnea, dysphagia, and chest pain after a high energy collision to the chest. This should raise suspicion for several common conditions such as rib fractures, pneumothorax, and hemothorax. A relatively rare and potentially life-threatening complication of chest trauma is posterior sternoclavicular (SC) dislocation. Posterior SC dislocation is caused by a direct force over the anteromedial portion of the clavicle or an indirect force over the posterolateral shoulder. It most often occurs due to high energy collisions such as motor vehicle accidents or sports injuries (football, rugby, hockey). Patients present with pain and deformity over the anteromedial aspect of the clavicle. In some cases, compressive symptoms may also be present. These include dyspnea, dysphagia, diminished pulses, venous congestion, or neurological symptoms. SC dislocations are difficult to appreciate on plain radiographs of the chest. If there is suspicion of a SC dislocation CT of the chest should be obtained. The management of posterior SC dislocation depends on the degree of dislocation and presenting symptoms. If there are signs of airway compromise, closed reduction should be performed immediately. In all other patients, reduction is typically performed in the operating room by an orthopedic surgeon. Cardiothoracic surgery should also be available due to the potential for complications such as bleeding from a vascular injury.

Workup : CT scan of the chest

Management : Urgent closed reduction if there are signs of airway compromise. Otherwise, reduction should occur in the operating room by an orthopedic surgeon with cardiothoracic surgery backup

Miscellaneous : Sternoclavicular dislocation is uncommon in young athletes as the growth plate of the medical clavicle remains open until 25 years of age.

Management: Endoscopic removal

Explanation

Foreign body ingestion is common in the pediatric population, particularly among children under the age of five years. The diameter of the esophagus in young children is small and predisposes to foreign body impaction. Button battery ingestion is associated with significant complications, especially if esophageal impaction occurs. Damage to the mucosa occurs due to electrical discharge, leakage of alkaline contents, and pressure necrosis. Ingestion is often witnessed however patients may present with non-specific complaints. If ingestion is witnessed it is important to determine the battery type, number of batteries ingested, time of ingestion, and magnet coingestion. Presenting complaints may include cough, vomiting, hematemesis, abdominal pain, or chest pain. Plain radiographs should be ordered to evaluate the size of the battery and anatomic location. Button batteries appear as a double ring due to their bilaminar structure. The management of button battery ingestion depends on the anatomic location and presence of symptoms. If the battery has passed the stomach, it will often pass through the gastrointestinal tract without complication. Patients can be discharged home with instructions to seek immediate medical attention if symptoms develop. If the battery is located in the stomach, urgent endoscopic removal is indicated (within 12 hours). In case of esophageal impaction, emergent endoscopic removal is indicated. Sucralfate should be given to neutralize the esophageal pH until the battery can be removed.

Workup : Chest X-ray

Management : Endoscopic removal

Miscellaneous : Even after removal of a button battery from the esophagus there may be ongoing injury for weeks and there is a risk severe sequela such as aortoesophageal fistula.

Explanation

Esophageal perforation is a rare gastrointestinal emergency. The most common etiology is iatrogenic perforation. It may also me caused by trauma, foreign body, tumor, or increased intraesophageal pressure (Boerhaave syndrome). Most patients present with chest pain which is described as severe, sudden onset retrosternal chest pain that may radiate to the neck, shoulder, or back. The pain is often exacerbated by inspiration and swallowing. Other presenting symptoms include dysphagia, dysphonia, hoarseness, shortness of breath, and vomiting. 50% of patients present with the classic triad of chest pain, vomiting, and subcutaneous emphysema (Mackler’s triad). Imaging is often necessary to confirm the diagnosis of esophageal perforation. Chest X ray findings are nonspecific and may include pneumomediastinum, pneumothorax, pleural effusion, and subcutaneous emphysema. In stable patients who can tolerate oral intake, an esophagram with water soluble contrast is the initial study of choice. Management of esophageal perforation in the emergency department includes NPO, intravenous fluids, broad spectrum antibiotics, and emergent surgical consultation.

Miscellaneous :Antibiotics should cover both aerobic and anaerobic organisms (piperacillin/tazobactam monotherapy or ceftriaxone + metronidazole or clindamycin)

Answer

Diagnosis: second degree AV block (Mobitz) type II
Management: Atropine, cardiac pacing

Explanation

Mobitz type II is a form of second-degree atrioventricular block characterized by constant PR interval and intermittent non conducted P waves (dropped beat). Mobitz type II often occurs in the setting of underlying structural heart disease. Causes include cardiomyopathy (sarcoidosis), myocardial ischemia, endocarditis, myocarditis, hyperkalemia, and AV nodal blocking drugs. The clinical presentation is variable however patients are usually symptomatic. Symptoms may include chest pain, shortness of breath, light-headedness, dizziness, syncope, fatigue, and sudden cardiac arrest. Diagnosis is established with a 12-lead electrocardiogram.

EKG features of Mobitz II include 

  • Intermittent non-conducted P waves 
  • PR interval is constant in the conducted beats 
  • No progressive prolongation of the PR interval (as seen in Mobitz I) 
  • QRS complex is usually wide (can be narrow in some cases)

Management depends on the severity of symptoms. A decrease in heart leads to decreased cardiac output and hypotension. This can result in shock, altered mental status, and myocardial ischemia. Patients with such symptoms should be treated urgently with atropine and transcutaneous cardiac pacing should be initiated. In patients without signs of hemodynamic compromise, transcutaneous pacer pads should be placed, and the patient monitored for signs of deterioration. In both cases, the etiology of the atrioventricular block should be investigated and corrected if possible. If no underlying cause is found, or if the etiology is irreversible, a permanent pacemaker is placed.

Workup : electrocardiogram

Management : atropine, transcutaneous pacing, cardiology consultation

Miscellaneous : In Mobitz type II, 80% of patients have block located in the bundle branches and 20% have a block located in the bundle of His.

Explanation

Orbital cellulitis is an infectious process that involves the orbital soft tissues. It occurs more frequently in children and is often a complication of bacterial rhinosinusitis. Other causes include orbital trauma, odontogenic infection, dacryocystitis, and ophthalmic surgery. Clinical features of orbital cellulitis include orbital pain, eyelid edema, proptosis, ophthalmoplegia, chemosis, and fever. It is important to distinguish between preseptal cellulitis and orbital cellulitis. Preseptal cellulitis does not involve the extraocular muscles and other orbital contents. It does not present with vision impairment, pain with eye movements, proptosis, and ophthalmoplegia. The diagnosis of orbital cellulitis can be established based on the clinical presentation. Imaging can be used to confirm the diagnosis but may not be needed routinely. Contrast enhanced CT or MRI of the orbits and sinuses are the imaging modalities of choice. Empiric intravenous antibiotics are used in all cases of orbital cellulitis. The antibiotics of choice are vancomycin plus ceftriaxone, cefotaxime, piperacillin-tazobactam, or ampicillin-sulbactam. Metronidazole should be added for anerobic coverage if intracranial extension is suspected. Early ophthalmology consultation is also recommended. Surgical management may be necessary in cases of intracranial extension or lack of improvement with intravenous antibiotics.

Workup : Contrast enhanced CT or MRI of the orbits and sinuses

Management : Empiric intravenous antibiotics

Miscellaneous : Fungal infection should be considered in immunocompromised patients. Orbital cellulitis may be related to mucormycosis or invasive aspergillosis.
