A 4-year-old boy is brought to the emergency department for shortness of breath 

  • She had a cough that started one week ago and progressively worsened. She developed shortness of breath 12 hours ago 
  • Associated symptoms include nasal congestion, low-grade fevers, and decreased appetite

Diagnosis:

Croup

Management:Inhaled racemic epinephrine, dexamethasone, supportive care

Explanation

Croup (acute laryngotracheobronchitis) is an acute viral syndrome characterized by subglottic and laryngeal edema that result in hoarseness, inspiratory stridor, and barking cough. It is most common in the winter and typically affects children between 6 months and 3 years of age. The majority are related to parainfluenza virus, with the remaining caused by other respiratory viruses (adenovirus, influenza virus, respiratory syncytial virus). Croup is usually preceded by a prodromal phase of acute upper respiratory illness (congestion, rhinorrhea, fever). Patients then develop a barking cough, often described as “seal-like”. This is accompanied by inspiratory stridor, hoarseness, and constitutional signs and symptoms. The severity of croup and associated respiratory distress varies between patients. In severe cases patients may have both inspiratory and expiratory stridor, hypoxia, cyanosis, and respiratory muscle fatigue.

Croup is a clinical diagnosis based on the history and physical examination. Radiographs of the chest characteristically show subglottic narrowing referred to as steeple sign. Management of croup depends on the severity of illness. In mild cases supportive care is the cornerstone of treatment. Corticosteroids (dexamethasone) can be used to reduce airway inflammation. Severe cases that involve stridor and/or respiratory distress should be treated with inhaled racemic epinephrine in addition to corticosteroids and supportive care. Endotracheal intubation is required in a minority of cases due to established or impending airway compromise.

Workup : Clinical diagnosis. Chest radiograph aids in diagnosis.

Management :Inhaled racemic epinephrine, dexamethasone, supportive care

Miscellaneous : Signs of impending respiratory failure include stridor at rest, lethargy, dusky appearance, and altered level of consciousness.

Explanation

Septic arthritis is an infection of the synovial fluid and joint tissues. It is an orthopedic emergency that requires prompt recognition and management. Routes of infection include hematogenous spread of bacteria into the joint space, direct entry through penetrating trauma, or contiguous spread from adjacent osteomyelitis. The most common cause of pediatric septic arthritis in all age groups is staphylococcus aureus. Haemophiles influenza, Escherichia coli, and group B streptococci are other common pathogens. Clinical features include reduced mobility, limping, fever, fussiness, and decreased appetite. Infants with septic arthritis of the hip may have a history of crying during diaper change as this involves manipulation of the lower extremities. The affected leg is held in flexion, abduction, and external rotation as this maximizes the joint space volume and reduces pain. Joint tenderness, swelling, erythema, and warmth are common examination findings. If there is suspicion for septic arthritis, the affected joint should be aspirated. Synovial fluid WBC count greater than 50,000/ml suggests septic arthritis. The fluid should also be sent for culture. Imaging has a limited role on the diagnosis of septic arthritis however should be used to rule out other causes of joint pain. Septic arthritis should be managed in conjunction with orthopedic and infectious disease specialists when possible. Antibiotics and joint drainage are the cornerstones of management. Antibiotic therapy should be started as soon as possible. Initial management can be guided by gram stain, age, and risk factors. Children < 3 months should initially be treated with an antistaphylococcal agent (oxacillin, vancomycin) plus ceftazidime, cefotaxime, or cefepime. Children > 3 months should be treated with an antistaphylococcal agent (vancomycin, clindamycin). Septic arthritis of the hip often requires arthrotomy and open drainage to avoid permanent joint damage.

Workup:

CBC, CRP, ESR, synovial fluid studies

Management:

Joint drainage (aspiration, arthrotomy) and empiric antibiotics

A delay in the diagnosis and management of septic arthritis of the hip can lead to long term complications such as femoral neck deformity, limb-length discrepancy, osteonecrosis, femoral head destruction, and growth disturbance.

A 13-year-old male is brought to the emergency department due to abdominal pain

  • The pain has been gradually worsening over the past day. It is described as epigastric, constant, and non-radiating 
  • Associated with multiple episodes of non-bloody, non-bilious vomiting 
  • He was involved in a bicycle accident where he fell forward onto the handlebars three days ago
  • Examination is remarkable for abdominal distension
  • CT scan with contrast reveals gastric distension and narrowing of the second portion of the duodenum
What is the most likely diagnosis and its management?

Diagnosis: duodenal hematoma

Management: NG tube, NPO, parenteral nutrition

Explanation

Duodenal hematoma is an uncommon complication of blunt abdominal trauma that most commonly seen in children. It is classically associated with a handlebar injury but can also occur in the setting of child abuse, motor vehicle collisions, falls, and sports related injuries. The abdominal organs in pediatric patients are more vulnerable to injury due to thinner abdominal muscles, more compliant ribs, and horizontal orientation of the diaphragm leading to increased organ exposure. Duodenal hematoma usually presents in the days following an abdominal injury. Symptoms include vomiting, abdominal pain, abdominal distension, and anorexia. External signs of trauma are not always evident. Imaging is necessary for a definitive diagnosis. It can be diagnosed with upper GI series or a contrast enhanced CT scan. Water soluble contrast should be used if perforation is suspected. Duodenal hematomas can be managed conservatively with nasogastric tube placement and parenteral nutrition. Ultrasonography can be used to follow a resolving hematoma. If symptoms do not resolve operative management may be necessary.

Workup:

Upper GI series or contrast enhances CT

Management:

Conservative in majority of cases with nasogastric tube placement and parenteral nutrition.

What is the most likely diagnosis and its management?

Answer:

Diagnosis: Fournier’s gangrene

Management: Broad spectrum antibiotics and surgical debridement

Explanation

Fournier’s gangrene is a rapidly progressive necrotizing infection of the perineum and genital region. As with other necrotizing infections, the infection spreads along the superficial and deep fascial planes. The overlying skin is usually spared, and the extent of infection is often difficult to appreciate. Fournier’s gangrene is a polymicrobial infection. The most common organism isolated are streptococcal species, staphylococcal species, anaerobic organisms, and Enterobacteriaceae. There are various routes of entry for infection including the urogenital tract, skin of the genitalia, and anorectum. Common causes of Fournier’s gangrene are perianal infections, anal fistula, urethral injury, urethral manipulation, prostate biopsy, urinary tract infections, epididymitis, scrotal injury, orchitis, and hidradenitis. It is more common in patients with impaired immunity due to diabetes, alcoholism, old age, HIV infection, malignant, immunosuppressive therapy, or alcoholism.

The presentation of Fournier’s gangrene is variable and can develop insidiously or rapidly. A prodromal phase consisting of fever and lethargy may occur up to a week prior to the onset of pain. The pain is typically severe and associated with edema of the overlying skin. In the initial stages the pain is often out of proportion to the physical exam findings. Eventually erythema of the skin develops, followed by subcutaneous crepitations and eventually gangrene with purulent drainage from wounds in the perineum and/or genitalia. Constitutional symptoms including fever, loss of appetite, malaise, and nausea are often present.

The diagnosis is established clinically based on the history and examination. Laboratory studies and Imaging such as computed tomography (CT) or ultrasonography should be obtained to evaluate the extent of infection. Fournier’s gangrene is a urologic emergency associated with a high mortality rate if not treated rapidly and appropriately. After initial stabilization, the cornerstones of therapy are broad spectrum antibiotic therapy and urgent surgical debridement of the necrotic tissue. Initial antibiotics should cover all of the potential causative organisms. Common antibiotic choices are ampicillin-sulbactam, piperacillin-tazobactam, or ticarcillin-clavulanate in addition to clindamycin and vancomycin.

Workup:

Clinical diagnosis. Imaging (CT, US) to evaluate extent of infection.

Management:

Broad spectrum antibiotics and surgical debridement

Diagnosis: Illness anxiety disorder

Management: Cognitive behavioral therapy

Explanation

Illness anxiety disorder (IAD), formerly known as hypochondriasis, is a psychiatric condition characterized by excessive fear of or preoccupation with having or developing a serious medical condition. The etiology of IAD is not known. It is associated with underlying anxiety disorders such as generalized anxiety disorder and panic disorder. Patients may have a personal or family history of severe illness during childhood.

IAD presents with severe, often disabling preoccupation with having a serious medical disorder despite a normal workup, few or mild somatic symptoms, and reassurance. Patients with IAD often seek advice from several physicians as they believe that their serious condition was missed. Common and benign symptoms such as occasional fasciculations or stomach rumbling are interpreted as signs of a serious medical condition.

The DSM-V criteria for illness anxiety disorder are as follows:

  • Excessive worry about having or developing a serious illness
  • Absence of somatic symptoms. If present, the symptoms are mild
  • Excessive concern and anxiety regarding health-related issues
  • Disproportionate and redundant health related behaviors 
  • Symptoms have been present for at least 6 months 
  • Symptoms are not better explained by another psychiatric condition

Cognitive behavioral therapy is first-line treatment for IAD. It involves behavioral modifications and education about normal somatic sensations. Pharmacologic therapy is second line. selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are the preferred initial agents.
