A 19-year-old male is brought to the emergency department after a motor vehicle collision 

  • His vehicle was struck head on at approximately 40 mph 
  • Patient was restrained in the passenger seat. His only complaint is left leg pain. He was given 50 mcg of fentanyl by EMS 
  • Vital signs are T 98.3 F, heart rate 96/min, blood pressure 138/78 mmHg, and respiratory rate 19/min 
  • On examination he is alert, oriented, and in no acute distress. The left leg is internally rotated, flexed, and adducted. It is slightly shorter than the right leg. Peripheral pulses are 2 + bilaterally and sensation is intact.
What is the most likely diagnosis and its management?

Answer:

Diagnosis: Posterior hip dislocation

Management: Imaging to confirm diagnosis, closed reduction

Explanation

Posterior hip dislocation refers to posterior dislocation of the femoral head relative to the acetabulum. The hip joint is inherently stable, and dislocation usually requires significant force such as high-speed motor vehicle collisions (MVCs). The mechanism of injury is hip flexion, adduction, and internal rotation. In MVCs posterior hip dislocation can occur when the knee strikes against the dashboard. Due to the force required to dislocate the hip, patients often present with other serious injuries. Dislocation of the femoral head can injury the femoral neurovascular bundle. Avascular necrosis of the femoral head can occur within 6 hours of dislocation.

Presenting complaints include severe pain and inability to bear weight on the affected extremity. The leg is classically shortened, internally rotated, adducted, and flexed at the hip. This positioning may not be present in patients with associated femoral shaft fractures. A complete neurovascular examination should be performed. Pelvic radiographs are usually sufficient to detect hip dislocations. On plain films the femoral head is usually displaced posterior, superior, and lateral to the acetabulum. Computed tomography (CT) should be obtained if there is an associated acetabular fracture. Due to the risk of avascular necrosis, reduction should be performed urgently (ideally within 6 hours of injury). Indications for open reduction include unstable fracture dislocation, delayed presentation (>12 hours), and irreducible dislocation.

Explanation

Meningococcal meningitis is a rare and potentially fatal infection caused by Neisseria Meningitidis. The incidence rate is 0.11 per 100,000 persons in the United States. The clinical presentation is variable and ranges from undifferentiated febrile illness to septic shock. Classically patients present with fever, altered mental status, and nuchal rigidity. Associated symptoms include headache, photophobia, seizures, petechial rash, nausea, and vomiting. Kernig’s and Brudzinski’s signs are frequently positive in meningitis. A positive Kernig’s sign is inability to extend the leg while the hip is flexed to 90 degrees. A positive Brudzinski’s sign is involuntary flexion of the hip and knee with passive flexion of the neck. Meningococcal septicemia can lead to Waterhouse-Friderichsen syndrome (acute hemorrhagic necrosis of the adrenal glands) characterized by rapidly developing septic shock and disseminated intravascular coagulation (DIC). The diagnosis of meningitis is confirmed with cerebrospinal fluid (CSF) studies; however, this should not delay empiric antimicrobial therapy. The choice of antibiotic depends on the patients age and most likely pathogen. In the age group of 2-50 years, the most common pathogens are N. meningitidis and S. Pneumoniae. Ceftriaxone and vancomycin are the preferred empiric antibiotics in this age group. Antibiotics may be deescalated based on the results of the CSF culture.

Workup:

CBC, metabolic panel, blood cultures, CSF studies

Management:

Broad spectrum antibiotics (vancomycin and ceftriaxone)

Explanation

This patient is presenting with signs and symptoms suggestive of classic heat stroke (severe nonexertional hyperthermia). Heat stroke is the most serious heat-related illness and is characterized by severe hyperthermia and central nervous system dysfunction. Nonexertional heat stroke typically occurs in the elderly but can occur in anyone with medical, physiological, or social risk factors that predispose to heat-related illness due to decreased thermoregulatory capacity. The diagnosis is established clinically based on a history of heat exposure, hyperthermia > 40°C (104°F), and neurologic abnormalities. Hypotension, tachycardia, and tachypnea are common. The skin is typically dry in nonexertional heat stroke due to the decreased sweat gland response in the elderly. Flushing, oliguria, pulmonary crackles, and altered mental status may also be present. Heat stroke is treated symptomatically and conservatively. Temperature reduction is the cornerstone of management. Evaporative and convective cooling methods are preferred in the elderly. These include application of ice packs fanning, and infusion of cold intravenous fluids. Antipyretics (NSAIDs, acetaminophen) have no role in the management of heat stroke.

  • Associated with decreased oral intake and fever with a maximum temperature of 102.5F 
  • Mother tried applying a moisturizer today and noted that the skin became “raw” 
  • On examination there is diffuse blanchable skin erythema. Scattered flaccid bullae are present over the trunk and extremities. The oral mucosa and conjunctiva are normal.
What is the most likely diagnosis and its management?

Diagnosis: Staphylococcal scalded skin syndrome

Management: Supportive care, penicillinase resistant antibiotics (nafcillin, oxacillin)

Explanation

Staphylococcal scalded skin syndrome (SSSS) is a potentially life-threatening toxin-mediated dermatologic disease that primarily affects infants and children. It is caused by exotoxin-producing strains of staphylococcus aureus. The exotoxins responsible for SSSS include exfoliative toxin A and exfoliative toxin B. These toxins cleave desmoglein 1 leading to disruption of the keratinocyte attachments in the epidermis. SSSS may be preceded by a staphylococcal infection (pharyngitis, bullous impetigo) although there is often no clinical signs of preceding infection. Initial symptoms are non-specific and include fever, irritability, poor appetite, and erythema of the skin. Within 48 hours flaccid bullae form which easily rupturing revealing a an erythematous base that gives it a “scalded” appearance. The skin tends to slough easily in response to mechanical pressure (Nikolsky sign). There is no mucous membrane involvement in SSSS (in contrast to other bullous conditions such as Stevens-Johnson syndrome and Toxic epidermal necrolysis). The diagnosis of SSSS is made based on clinical findings. Cultures of suspected sources (skin, nasopharynx) can be obtained to support the diagnosis although these may be negative. A skin biopsy should be obtained if the diagnosis is unclear and other causes of bullous skin disease are suspected. SSSS is managed with supportive care anti staphylococcal antibiotics. Patients with severe disease may require admission to a burn unit. penicillinase resistant antibiotics are preferred (nafcillin, oxacillin). Vancomycin should be used in areas with high prevalence of community acquired MRSA.

Workup:

Diagnosis is established based on clinical findings. Cultures can support the diagnosis

Management:

Supportive care, penicillinase resistant antibiotics (nafcillin, oxacillin)

Children are much more susceptible to SSSS compared to adults. This is thought to be related to impaired renal excretion of exotoxins and lack of antibodies against staphylococcal exotoxins.
