What is the most likely diagnosis and its management?

Answer

Diagnosis: Indirect inguinal hernia
Management: Surgical correction

Explanation

Inguinal hernia (IH) refers to a protrusion of intraabdominal contents through a defect in the myofascial plain of the oblique and transversalis muscles. IH is one of the most common surgical pathologies. IH is classified as either direct (33%) or indirect (67%). Indirect IH occurs when the abdominal contents protrude through the inguinal ring into the inguinal canal. Direct IH is protrusion of the abdominal contents through the transversalis fascia within Hesselbach’s triangle.
Increased intraabdominal pressure contributes to the development of IH. Risk factors include obesity, chronic cough, straining due to constipation, and heavy lifting. IH typically presents with a bulge in the inguinal region. Patient may report pain at the time of presentation. The size of the bulge and severity of pain tend to increase with increased intraabdominal pressure (coughing, lifting, etc.). Severe constant pain and obstructive symptoms are rare and suggest incarceration or strangulation.
A thorough physical examination is often all that is required to make the diagnosis of IH. The hernia may be obvious on inspection. The size of IH decreases when changing from the standing to supine position and increases with coughing. evaluation of the inguinal canal is performed by palpating through the scrotum towards the external inguinal ring. A protrusion in the scrotum suggests an indirect IH. In equivocal cases, imaging such as ultrasound or CT should be performed to confirm the diagnosis.
Management of IH is usually surgical. In patients with small and asymptomatic hernias, watchful waiting is acceptable.

Workup : Clinical diagnosis

Management : Surgical correction

Miscellaneous : Indirect inguinal hernias are found lateral to the inferior epigastric vessels, and direct inguinal hernias lie medial to them.

Explanation

Social anxiety disorder (SAD) is characterized by excessive and persistent fear of embarrassment and humiliation during social interactions that leads to social avoidance and impairment. It is a common disorder with a lifetime prevalence of 5-10% of the population. It affects women more often than men and the most common age group is 15-25 years old.

Patients with SAD have an irrational fear of various social and performance situations. They generally have good insight into their condition. Avoidance of social activities is common, particularly if there is a high chance that they will be the focus of attention (interactive classes, presentations). Physical manifestations of SAD include palpitations, sweating, blushing, and trembling. Performance-only social anxiety disorder refers to distressing symptoms and fear related to performance, such as giving speeches or presentations. The symptoms occur prior to or during the anxiety causing event and resolve shortly after. Anxiety related to other social situations is absent in performance only SAD.

Management of performance-only SAD consists of both pharmacotherapy and psychotherapy. Cognitive behavioral therapy is the preferred psychotherapy. A beta-blocker such as propranolol taken 30 minutes prior to the anxiety causing event is the treatment of choice. Benzodiazepines can also be used however they have undesirable adverse effects such as sedation and addiction potential.

Workup : Clinical diagnosis

Management : Propranolol, cognitive behavioral therapy

Miscellaneous : Performance-only social anxiety disorder presents with distressing symptoms related to specific anxiety causing events. Treatment with an as needed medication such as propranolol is often sufficient to manage the symptoms.

Explanation

Ramsay Hunt syndrome (RHS), also referred to as herpes zoster oticus, is an otologic complication of varicella-zoster virus (VZV) reactivation that causes inflammation of the geniculate ganglion of the facial nerve (CN VII). It rarely affects other cranial nerves such as CN V, IX, and X. RHS is rare, with an incidence of approximately 5 per 100,000 individualsper year. It can affect patients of all ages but is most common in those in the 7th and 8th decades of life.
RHS classically presents with the triad ofvesicles in the auditory canal, otalgia, and ipsilateral facial paralysis. A prodrome of fever, pain, and fatigue may occur. The lesions initially appearas erythematous papules and progresses to vesicles which rupture and crust over within one week. RHS may also cause tinnitus, vertigo, hearing loss, hyperacusis, dysarthria, and hoarseness. RHS is a clinical diagnosis. Tzanck smear or PCR analysis of vesicular fluid can confirm the diagnosis. Management of RHS consists of valacyclovir and systemic corticosteroids (prednisone). Supportive care includes analgesics, antiemetics, and artificial tears. Patients should be advised to tape the affected eye shut at night to prevent exposure keratopathy. Most patients recover neurologic function within one year. The prognosis of RHS is generally worse than that of Bell’s palsy.

Workup : Tzanck smear or VZV PCR

Management : Prednisone, valacyclovir, supportive care

Miscellaneous : Paralysis of the forehead muscles is helpful in distinguishing peripheral facial nerve lesions from central lesions. The forehead usually lift symmetrically with central facial nerve lesions due to bilateral cortical innervation of the frontalis muscle.

Diagnosis: Major depressive disorder
Management: SSRI, psychotherapy

Explanation

Major depressive disorder (MDD) is a common and debilitating disease characterized by depressed mood, anhedonia, impaired cognitive function, and vegetative symptoms. The lifetime prevalence of MDD is between 10% and 20%. The etiology is complex and involves genetic, psychological, and biological factors.

The criteria for major depressive disorder include either depression or anhedonia for two weeks in addition to five of the following symptoms: 

  • Depressed mood for most of the day, almost every day 
  • Sleep disturbance 
  • Feelings of worthlessness 
  • Anhedonia 
  • Fatigue or loss of energy 
  • Decrease concentration, cognition, and ability to make decisions 
  • Weight change 
  • Psychomotor retardation or agitation 
  • Suicidal ideation

The diagnosis is established clinically based on the above criteria. Further evaluation may be necessary to rule out organic etiologies such as hypothyroidism or structural brain disease. Management includes psychological therapy, behavioral therapy, and anti-depressants. Selective serotonin reuptake inhibitors (SSRIs) are the first line pharmacotherapy. Antidepressants generally require > 4 weeks to take effect. Once remission is achieved, therapy should be continued for an additional 6 months prior to slowly tapering off. Patients who have had multiple major depressive episodes require a longer maintenance phase. 

Workup : Clinical diagnosis

Management : SSRI, psychotherapy

The symptoms of depression can be remember using the mnemonic SIG-E-CAPS. Sleep changes, Interest (loss), Guilt (worthless), Energy (lack), Concentration/Cognition (reduced cognition or difficulty concentrating), Appetite (or weight change), Psychomotor retardation/agitation, Suicidal ideation

Explanation

MDMA (3,4-methylenedioxymethamphetamine), commonly referred to as “ecstasy”, is a stimulant that shares properties with amphetamines. It causes the release of catecholamines and inhibits their reuptake into presynaptic vesicles. MDMA is often abused by young partygoers, particularly at electronic dance music events or “raves”. The dose of MDMA can vary widely between pills, which increases the risk of toxicity.

The effects of MDMA include euphoria, increased alertness, peacefulness, and increased energy. Common adverse effects include bruxism, agitation, nausea, tachycardia, and hypertension. More serious adverse effects are seen with higher doses of MDMA. These include life-threatening tachycardia and hypertension, delirium, seizures, hepatotoxicity, hyperthermia, and hyponatremia. Hyponatremia is related to the consumption of large amounts fluids and persistent secretion of ADH that decreases the rate of water excretion. Severe hyponatremia can lead to seizures, coma, and death.

Evaluation of suspected MDMA intoxication should include urine drug screen, serum electrolytes, renal function tests, creatine kinase, glucose, transaminase levels, and coagulation studies. Management depends on the severity of symptoms. Evaluation of the ABCs (airway, breathing, and circulation) is the best first step. Patients may require endotracheal intubation in the case of hyponatremia induced obtundation. Severe hypertension is initially treated with benzodiazepines. Activated charcoal can be given if ingestion occurred less than one hour prior to presentation. Hyperthermia is treated with cooling measures such as cold-water immersion or cooling blankets. Hyponatremia is often mild and responds to fluid restriction. Hypertonic saline should be administered in the case of severe and symptomatic hyponatremia

Workup : urine drug screen, serum electrolytes, renal function tests, creatine kinase, glucose, transaminase levels, and coagulation studies

Management :Benzodiazepines, cooling blankets, hypertonic saline

Miscellaneous : MDMA is a commonly abused stimulant drug that can lead to severe hyponatremia due to excessive fluid intake and inappropriate secretion of antidiuretic hormone (ADH).
