Explanation

Atrial natriuretic peptide will be increased in response to increased blood volume. Atrial natriuretic peptide:

  • Released by atrial myocytes in response to increased blood volume and atrial pressure
  • Acts via cGMP 
  • Decreases sodium absorption in the kidneys 
  • Vasodilation of afferent renal arterioles and constricts efferent renal arterioles, causing diuresis

During a procedure, stress-induced secretion of ADH increases sodium retention in the kidneys and increases blood volume. During recovery, this increased blood volume increases atrial pressure, stimulating the release of atrial natriuretic peptide.

Tertiary syphilis:

  • Antibiotics for syphilis

What is the congenital heart disease that may cause this?

Answer

Ebstein anomaly

  • The tricuspid leaflets are displaced into the right ventricle “atrializing” the RV.
  • Associated with maternal lithium use
  • Often has tricuspid regurgitation, right-sided heart failure, and accessory conduction pathways

Differential Diagnosis: The “5 T’s” and Ebstein anomaly

Truncus arteriosus (1 vessel) The truncus arteriosus does not divide into the pulmonary trunk and aorta due to failure of the aortopulmonary septum to form.

Transposition (2 vessels) The aorticopulmonary septum fails to spiral, resulting in the aorta originating from the right ventricle and the pulmonary trunk originating from the left ventricle.

Tricuspid Atresia (Tri=3) Absence of the tricuspid valve and a hypoplastic RV. Incompatible with life unless there is both an ASD and VSD.

Tetralogy of Fallot (Tetra=4) Anterior displacement of the infundibular septum. Includes:

  • Pulmonary infundibular stenosis
  • Right ventricular hypertrophy
  • Overriding aorta
  • VSD

TAPVR Total Anomalous Pulmonary Venous Return (5 letters in the name) Pulmonary veins drain into the right heart circulation. Right to left shunting to maintain cardiac output occurs through an ASD and sometimes a PDA.

Ebstein Anomaly Tricuspid valve leaflets displaced into the RV, “atrializing” the RV. Tricuspid regurgitation, right-sided heart failure, and accessory conduction pathways are all associated with the condition. Associated with prenatal lithium use.

Loss of stretch in what areas of the cardiovascular system allowed him to maintain his blood pressure?

Answer

The aortic arch and carotid sinus baroreceptors respond acutely to changes in blood pressure.

Acute decreases in blood pressure are reversed by vasoconstriction, increased heart rate, and increased contractility.

Hypotension-> decreased stretch-> decreased afferent baroreceptor firing-> increased sympathetic efferent firing, decreased parasympathetic firing-> vasoconstriction, increased heart rate, increased contractility, increased blood pressure

Acute blood loss stimulates vasocontraction and an increased heart rate. In a young healthy person, there may be little or no decrease in blood pressure, even with significant blood loss. The heart rate increases to compensate for the loss in blood volume and cardiac output. Tachycardia may be the only sign of significant hemorrhage until blood loss is significant, then blood pressure can decrease rapidly. The aortic arch baroreceptor sends signals via the vagus nerve to the solitary nucleus of the medulla.

The carotid sinus signals transmit via the glossopharyngeal nerve to the solitary nucleus of the medulla.

  • Rhythm is irregularly irregular with no discrete P waves on ECG 
  • Contrast with multifocal atrial tachycardia with at least 3 distinct P wave morphologies

Risk factors: 

  • HTN 
  • CAD 
  • Diabetes 
  • Smoking 
  • CHF 
  • hyperthyroidism

Treatment: 

  • Rate and rhythm control 
  • Cardioversion 
  • Consider anticoagulation based on stroke risk
  • Definitive treatment is ablation of pulmonary vein ostia or left atrial appendage

The function of potassium during phase three of the action potential of cardiac myocytes is to efflux from the cell leading to a repolarization of the action potential. Phase three of the action potential is the initial repolarization which is completed at phase four.

Epidemiology:

Occurs in cardiac myocytes except SA and AV nodes.

Phase 4, resting membrane potential:
Inward rectifier K+ channels are fully open, resetting resting membrane potential at -85 mV.

Phase 0, rapid depolarization:
Voltage-gated Na+ channels open and inward Na+ current depolarizes cell membrane, which closes inward rectifier K+ channels.

Phase 1, initial repolarization:
Voltage-gated Na+ channels are inactivated and voltage-gated K+ channels open.

Phase 2, plateau:
Inward Ca2+ current balances outward K+ current. Ca2+ influx causes release of Ca2+ from sarcoplasmic reticulum (Ca2+ induced Ca2+ release), resulting in myocyte contraction.

Phase 3, repolarization:
Voltage-gated Ca2+ channels close and voltage-gated slow rectifier K+ channels open, causing K+ efflux.
