Waterhouse-Friedrichsen syndrome is most often a complication of Neisseria Meningitis in children and asplenic patients. It causes bilateral adrenal gland hemorrhage which leads to acute adrenal insufficiency. Other causes of Waterhouse-Friedrichsen syndrome are less common but include low platelet count, antiphospholipid syndrome, renal vein thrombosis, and corticosteroid use.

The patient most likely has a toxic multinodular goiter. The painless goiter with enlarged nodules in an elderly woman with symptoms of hyperthyroidism raises the suspicion for toxic multinodular goiter. The histology shows distended follicular cells with increased colloid and decreased epithelium. These findings are strongly indicative of toxic multinodular goiter.

Pathophysiology:

  • Hyperactive areas of thyroid produce excess T4 and T3 → ↓TSH → atrophy of other areas of thyroid

Epidemiology:

  • More common in elderly women. Rarely malignant.

Diagnosis:

  • Patchy uptake on radioiodine scan
  • Multiple nodules contained within the thyroid parenchyma

Histological Findings:

  • Patches of distended follicular cells with increased colloid and decreased epithelium

Answer

The histological features and image are indicative of papillary carcinoma. It is the most common type of thyroid cancer and is marked by psammoma bodies and “orphan Annie” nuclei.

Epidemiology:

  • Most common type of thyroid cancer (75-80% of cases).
  • Peak incidence is seen in patients between 30 and 50 years of age
  • Well-differentiated cells and therefore associated with a very good prognosis.
  • Associated with RET/PTC rearrangements, BRAF mutations, and exposure to ionizing radiation, especially during childhood (i.e. as radiation therapy for another type of cancer such as lymphoma).

Clinical Features:

  • May be asymptomatic
  • Local effects may result in hoarseness, dysphagia, dyspnea, or rarely, Horner syndrome or venous obstruction (e.g. SVC syndrome)

Histological Findings:

  • Large, empty-appearing nuclei with central clearing (“Orphan Annie” nuclei),
  • Psammoma bodies (concentric calcifications),
  • Nuclear grooves
  • Thyroglobulin tumor marker
